Emergency Medical Authorization Form - This form provides contact information for the coaches/club in the event of an emergency with your son or daughter. It also authorizes the coach/club to seek emergency medical treatment if necessary. This is one of two forms that are required to be completed and turned in to club/coach prior to any player being allowed to participate in lacrosse practices or games.
Mason Lacrosse Club Grievance Procedure - This document (adopted by the Mason Lacrosse Board in February, 2008) outlines the steps a parent/guardian should take to address any problem or complaint they may have with a coach.
Mason Lacrosse Medical Waiver Form - This form verifies that your player has had a valid physical exam and has received permission from the doctor to participate in lacrosse practices, games & activities.
