Prior to the start of any lacrosse practice or game (optional or official season), each player MUST submit the following two forms to the coaching staff. 

YOUR CHILD WILL NOT BE ABLE TO PARTICIPATE IN ANY PRACTICE OR GAME 
UNTIL THESE TWO FORMS ARE IN THE COACHING STAFF'S POSSESION.


1.  Emergency Medical Authorization Form
This form provides contact information for the coaches/club in the event of an emergency with your son or daughter.  It also authorizes the coach/club to seek emergency medical treatment if necessary.  
Please download, and fill out this standard form from the Mason Lacrosse "Documents" tab or click on the "highlighted form in blue letters" above. This form verifies that your player has had a valid physical exam and has received permission from the doctor to participate in lacrosse practices, games & activities.

Mason Lacrosse Club has developed a standard Medical Waiver Form to be used by all players as of the 2011 season.  Please download and use this standard form from the Mason Lacrosse "Documents" tab or click on the "highlighted form in blue letters" above.

During this transition year, the club will accept other forms that show your son/daughter had a physical exam and has permission to participate in lacrosse.  Acceptable forms must meet the following requirments:

1.  Form must clearly state the Doctor's name, address and telephone # (Examples include the Doctors Office Letterhead Stationary or prescription pad).
2.  Form must clearly state the date the physical exam was performed.
3.  Form must be signed and dated by the doctor acknowledging that player was examined by said doctor and has the doctor's permission to participate in lacrosse activities with no restrictions.


To be valid for the entire 2011 season, the physical exam must have been completed any time after June 30, 2010.  Physical exams are valid for 1 year from the date of the exam.  If the physical provided to the coach/club expires during the season, an updated physical form must be submitted immediately.

NOTE:
There are a few facilities in the area that will do Walk-In Sports Physicals with no appointment necessary.

Doctor's Urgent Care
