Mason Lacrosse Club - Spring Season
- Copies of player’s birth certificate are collected and filed in the event our league adopts the age-based program structure. This has not occurred but we are being proactive. If your daughter is a returning player and you have already submitted her Birth Certificate, you will not be asked for it again. If your daughter is a new player or you have not submitted a Birth Certificate to the club yet, you will have the option of uploading a scanned version of the document when you register or you can mail a copy of the birth certificate to:

- During the registration process the Emergency Medical Authorization, Medical Information, Concussion/Injury Policy, Sudden Cardiac Arrest Policy (Lindsay's Law) and various waivers are filled out and acknowledged electronically during Part 2 of Registration so, no doctors paperwork is required.
